Observed visibly damaged/rotted ceiling areas in back storage area with daylight visible and rain water intrusion present. Repeat: 6-501.12 Floors, walls, ceilings including the attachments such as soap, towel dispensers; light fixtures; and heat/ac vent covers shall be clean. Caution should be used to minimize food exposure. Observed buildup and debris present on flooring under/behind equipment throughout back of house. Wet food buildup with foul odor present on flooring inside of WIC. 6-501.114 Discard items and litter stored on the premises as they are not necessary to operation and maintenance. Observed storage areas of establishment inaccessible due to heavy presence of accumulated unused equipment and other items. EHS is unable to access these spaces to assess cleaning, storage practices, facility structure condition, or pest control. Remove unused items.
