6-201.11 Floors, Walls and Ceilings - Cleanability (C). There are two ceiling tiles in kitchen that are porous. These type of tile absorb moisture and not easily cleansable.
6-501.11 Repairing - Premises, Structures, Attachments, and Fixtures - Methods (C). There is a missing floor tile in corner under dry storage rack, some debris have fallen into hole in floor. Replace so that floor is easily cleansable and to prevent pest harborage. Also in kitchen, ceiling tile in corner near refrigerator observed stained. Repair/replace ceiling tile.
6-501.12 Cleaning, Frequency and Restrictions (C). Kitchen door, cabinet doors under hand sink, cabinet under cereal station, floors in kitchen under refrigerator and storage rack have build up.
