Observed a prep unit holding half the food above 41F.The left side was holding hamburgers, chicken, hotdogs above 41F, the other side was 40F. CDI-Maintenance was on site and observed the unit was frozen at the fans, maintenance was fixing when EHS left and PIC said they will not use until it is fixed. PIC moved food to the WIC because they brought it out around 9am.

Observed grill bricks in the bathroom, PIC said they use it for cleaning the oven (they were in packages in a box) CDI-PIC voluntarily discarded.

Observed 2 prep units not working, frozen fans-maintenance was on site and repairing. No TCS foods left inside when EHS left. WIF door has no latch on the front and an unapproved door knob. Rusted shelves throughout facility. REPEAT.
