CDI - relocated to WIC to quickly chill.

Ambient air temperature of unit 43f. This unit was noted on previous inspection for holding TCS foods at elevated temperatures. Repair/service unit to ensure TCS foods are held at or below 41f. Use alternate cooler unit for holding TCS foods until the unit has been repaired. Verification required within 10 days.

PIC must use plyers to turn on/off hot water. Observed leak from drain line at only back of house hand sink. Repair. Observed missing plumbing drain caps on floor drain in womens restroom.

Urine on flooring in front of urinal in mens restroom with strong foul odor present. Urinal water supply has been turned off, however, fixture not marked out of service and continues to be used by customers with waste present and foul odor.

Observed missing and/or damaged ceiling tiles in back of house areas and in restrooms. Observed visibly damaged/rotted ceiling areas in back storage area with daylight visible and rain water intrusion present. REPEAT 6-501.12 Floors, walls, ceilings including the attachments such as soap, towel dispensers; light fixtures; and heat/ac vent covers shall be clean. Caution should be used to minimize food exposure. Observed buildup and debris present on flooring under/behind equipment throughout back of house. Wet food buildup with foul odor present on flooring inside of WIC. REPEAT 6-201.14 Floor Carpeting, Restrictions and Installation (C). Observed floor carpeting runner present on floor inside of womens restroom. Carpeting is prohibited inside of restrooms. 6-501.114 Discard items and litter stored on the premises as they are not necessary to operation and maintenance. Observed storage areas of establishment inaccessible due to heavy presence of accumulated unused equipment and other items. EHS is unable to access these spaces to assess cleaning, storage practices, facility structure condition, or pest control. Remove unused items.
