General CommentsGeneral Comment: #10. Replace dumpster with missing lid behind 600 building. Replace missing drain plug in dumpster near cafeteria area. #11. Repair damaged board on ramp/ walkway leading to modular unit. Address poison ivy growing under ramp leading to modular unit and growth on side of modular unit. Replace missing drain cover in yard area at end of modular unit. Repair or remove old green house and stuff from premises. Clean and organize gray building out back.

Observed gap along edge of wall and floor riser. Clean walls under hand drying devices in bathrooms. General cleaning of floor in boys bathroom throughout in building. Thoroughly clean corner of floor in boys bathroom in 400 building. Observed where rusted metal corner wall molding were painted in bathroom stalls and rust is coming through paint again. Address rusted metal corner wall molding in bathroom/stall(100, 300 and 400 building). Repair out of order toilet in girls bathroom on higher 100 hall. Replace missing rubber cove base molding in boy bathroom in 200 building. Repair ceiling in women bathroom located near concession stand at football field. Walls and ceilings of toilet facilities shall be constructed of non-absorbent, washable materials and shall be kept clean. 15A NCAC 18A .2408 (b). Repair damaged sinks in science rooms. Observed sink basin separating from science lab table. Observed leaking plumbing fixture on sink in science lab. Repair hand drying device in bathroom. Provide soap and paper towels to at least one sink in science labs. Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b). 6 30 FLOORS: WALLS: AND CEILINGS: Floors, walls, and ceilings clean and in good repair 30# (.2410) No No No No No Patch holes in walls around piping throughout in different building. Address gaps along wall and flooring throughout in different classrooms in different building. Replace water stained ceiling tiles throughout in hallway, classrooms and modular unit. Observed several ceiling tiles throughout in hallway, classrooms and modular units with microbial growth. Address any roof leaks, leaks around window, HVAC leaks; then replace water stained ceiling tiles and tiles with microbial growth. Place out of place ceiling tile back into place in 400 building. Seal area around exterior of window in 400 building if needed. Observed paint on wall around and near window in 400 building bubbling and flaking. Repair floor in modular units. Observed flooring and floor tiles throughout several modular units damaged. Monitor and repair walls in modular units if needed. Tightly secure loose rubber cove base molding in culinary class. Replace missing paneling, board or glass in gym window. Secure and repalce looseand missing tranisitonal molding in gym area. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.
9 35 DRESSING ROOMS AND SHOWERS: Floors, walls, and ceilings clean, in good repair, washable, non-absorbent 30#; fixtures clean, in good repair 40#; facilities for storage of clothes provided, kept clean 20#; soap and towel 10* (.2413) No No No No No Thoroughly clean floor and lower section of wall in dressing rooms/locker rooms. Secure loose rubber cove base molding at football field dressing room. Thoroughly clean wall urinal in gym dressing room/locker room and repair leaking toilet. Remove damaged couch from gym dressing room. Replace damaged floor drain cover in home team dressing room at foot ball field. 11 50 PREMISES: MISCELLANEOUS: Premises neat, clean 30#; no vector breeding or harborage 20#; pesticides and other toxic materials properly handled and stored 30* (.2415) No No No No No Observed roaches present in culinary class. Have room and school exterminated to address roach issue! Observed mice dropping throughout in 400 building and bleacher area in gym. Have exterminator to address any mice or rodent issues. PATCH HOLES IN WALLS AND AROUND PIPING. Monitor and address sink hole starting to form behind 400 building. Observed ant and roach spray throughout in different classrooms in different building. Highly recommend teachers not spraying or applying bait in rooms. Observed spray along flooring and white powder along flooring in several rooms. Address ant mound leading out to football field. There shall be no fly or mosquito breeding places, rodent harborages, or undrained areas on the premises. 15A NCAC 18A .2415 (a). Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b).
