General CommentsMr. Morrison (head custodian) was present during the inspection. NO POINTS WERE DEDUCTED FOR THE FOLLOWING: 7. Observed science prep rooms were in need of organizing and cleaning. 9. Observed some lockers were rusty and had debris, the varsity dressing rooms with debris on the floor, and a shower bench was damaged with exposed cushioning in the girls locker room shower. **Floors, walls, and ceilings of dressing rooms and showers shall be kept clean and in good repair. 15A NCAC 18A .2413 (a). **All fixtures shall be kept clean and in good repair. 15A NCAC 18A .2413 (d). **Adequate facilities for storage of clothes and other personal items shall be provided and kept clean. 15A NCAC 18A .2413 (e).

5 50 LAVATORY FACILITIES: Fixtures clean and in good repair 30#; soap and individual towels or approved hand-drying devices provided 40# (.2409) No No No No No Repeat Non-Compliance - 5a) Observed some sink faucets were pitted in the 8th grade restrooms, a sink fixture with very low water pressure and one fixture not working in the 6th grade girls restroom. 5b) Observed several paper towel dispensers were jammed (8th grade girls, 8th grade single restroom, 8th grade boys). Improvement observed; all sinks were supplied with soap and towels. **Repair or replace broken dispensers so that towels are accessible for hand washing. **Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).
6 30 FLOORS: WALLS: AND CEILINGS: Floors, walls, and ceilings clean and in good repair 30# (.2410) No No No No No Repeat Non-Compliance - Observed curtain dividing wall was damaged (worn / torn) in room 273, a floor electrical outlet was broken in the media center, carpeted areas were discolored and worn, the ceiling with moisture damage / discoloration in 7th grade hallway, the ceiling texture with flaking onto the floor, ceiling panels were not secure in MC488, MC487
8 20 LIGHTING AND VENTILATION: Fixtures, grills, vents, blinds, drapes, etc., clean and in good repair 40# (.2412) No No No No No Observed dead bugs and other debris in light fixtures throughout the school building; gym ventilation was dusty, and the window blinds were damaged in MC487. **Lighting and ventilation shall be provided and installed as required by the N.C. State Building Code. 15A NCAC 18A .2412 (a). **All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b).
11 30 PREMISES: MISCELLANEOUS: Premises neat, clean 30#; no vector breeding or harborage 20#; pesticides and other toxic materials properly handled and stored 30* (.2415) No No No No No Repeat Non-Compliance - Observed Clorox and other disinfectant wipes, aerosol air fresheners were stored on desks and counters accessible to the students. **Stored all cleaners and other hazardous products out of reach of children as directed on product labels. **Pesticides and other toxic materials shall be used as directed on the label and handled and stored as to avoid health hazards. 15A NCAC 18A .2415 (b).
