5 35 LAVATORY FACILITIES: Fixtures clean and in good repair 30#; soap and individual towels or approved hand-drying devices provided 40# (.2409) No No No No No Thoroughly clean hand washing sinks/sinks within classroom area. Observed trash/debris present in sinks. Provide soap and paper towels at hand washing sink in 200 and 300 boys bathroom. Fixtures shall be kept clean and in good repair. 15A NCAC 18A .2409 (b). Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).
6 15 FLOORS: WALLS: AND CEILINGS: Floors, walls, and ceilings clean and in good repair 30# (.2410) No No No No No Clean walls throughout in classrooms and repaint. Replace missing rubber cove base moulding in 119. Repair any roof leaks outside of room 206 and in room G2 and replace water stained ceiling tiles. Observed gaps along base of wall and floor in 100 building and 200 building classroom. Seal gaps along base of floor and wall. Replace any broken/damaged floor tiles in classroom area.******Immediately replace electrical outlet cover in B-13. Observed exposed wiring and outlet is pulled out of wall. Secure data entry lines within classrooms.*** Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.
7 10 STORAGE SPACES: Clean 20#; storage off floor 20# (.2411) No No No No No Clean and organize storage rooms/closets. Remove storage off the floor in book room, gym storage, and general storage rooms. All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411.
8 20 LIGHTING AND VENTILATION: Fixtures, grills, vents, blinds, drapes, etc., clean and in good repair 40# (.2412) No No No No No Replace broken window in 100 building and 400 building. Secure light shield in 201, 312. Thoroughly clean return air ventilation in 206, 224, 226, MC370, G2 and gym area. Wipe/clean top of lockers. Repair or remove broken furniture(chairs) from 317 and B13. All windows and fixtures (grills, vents, blinds, drapes, lighting fixtures, etc.) shall be kept clean and in good repair. 15A NCAC 18A .2412 (b).
9 15 DRESSING ROOMS AND SHOWERS: Floors, walls, and ceilings clean, in good repair, washable, non-absorbent 30#; fixtures clean, in good repair 40#; facilities for storage of clothes provided, kept clean 20#; soap and towel 10* (.2413) No No No No No Patch hole in wall/base of wall in boys dressing room. Thoroughly clean floor in dressing room in locked area. Repair damaged lock on back door to boys dressing room. Repair floor upon entering boys dressing room behind entrance door. Monitor and repair lockers in dressing room. Floors, walls, and ceilings of dressing rooms and showers shall be kept clean and in good repair. 15A NCAC 18A .2413 (a). 11 15 PREMISES: MISCELLANEOUS: Premises neat, clean 30#; no vector breeding or harborage 20#; pesticides and other toxic materials properly handled and stored 30* (.2415) No No No No No Observed old shelving behind 300 building and old rotten/damaged boards near dumpster area and open area at entrance to basement area under cafeteria. Remove old rotten/damaged boards and book shelving from premises. Clean grounds around/near dumpster area. The premises of the school under control of the principal shall be kept neat and clean at all times. 15A NCAC 18A .2415 (a).Waste material, unnecessary articles, rubbish, litter, or garbage shall not be allowed to accumulate on the premises. 15A NCAC 18A .2415 (a).
