Facility has switched to pre-made quat sanitizer spray bottles.

Italian sausage observed cooling in the prep sink at 98F. Vent containers of cooling foods to allow for rapid cooling. Utilize ice baths, smaller portions, or other effective methods to cool rapidly. CDI - Educational discussion. Lids were removed from the marinara sauce and Italian sausage was relocated to the walk-in cooler to cool rapidly.

Door handle has been replaced, incorrectly and PIC must do modifications with tape for door operate normally. 6-501.111 Controlling Pests (C) Repeat - Improvements noted. Only one live roach observed on the wall and a few flies observed in the facility. PIC switched pest control provider and a technician comes to treat every other week.

The door gaskets are crusted enough to obscure gasket texture and seams in some areas The racks in the walk in cooler are dusty. The exterior of the standing dough mixer is crusted with dry bits of dough and flour. The exteriors of bulk storage containers for dry goods and some sauces are stained or tacky to the touch. Clean at a greater frequency to preclude accumulation.

Extra cleaing needed throughout establishment including the FRP walls exhibiting staining and residue under equipment, shelving and in corners and hard to reach areas. 6-501.11 Repairing - Premises, Structures, Attachments, and Fixtures - Methods (C) There is a large crack running through the front of the kitchen floor and a smaller one at the bathroom. A tile under the 3 comp is damaged and holding water. Coving is peeling away from wall under the 3 comp. Repair damaged areas of the floors and walls and stained ceiling tiles.
