Observed back door opened a small amount. Protect outer openings of establishment from insect or rodent entry by screening openings or keeping doors/windows closed.
Keep all doors closed when not in use or provide a screen for the back door. Kept at half points for improvement. Door was opened only a small amount for a cross breeze. |

The front door was broken and falling off the hinges. It had to be pulled shut and wedged into the frame with both hands. Floors, walls, and ceilings including the attachments such as soap and towel dispensers; light fixtures; and heat/ac vents shall be maintained in good repair. Repair the door. |
