P During inspection today 1 pan of sausage was found on the flat top griddle holding 117F. This was found to be due to the pan holding the sausage being placed on a portion for the flat top burner not currently turned on. CDI by having PIC reheat the product to 165F or greater.
25 0.50 Consumer advisory provided for raw/ undercooked foods No No Yes 3-603.11 - Consumption of Animal Foods that are Raw, Undercooked, or Not Otherwise Processed to Eliminate Pathogens: Disclosure shall identify the animal-derived food by asterisking them to a footnote that states that the items are served raw or undercooked, or contain (or may contain) raw or undercooked ingredients. Pf 3-603.11 - Consumption of Animal Foods that are Raw, Undercooked, or Not Otherwise Processed to Eliminate Pathogens: Provide consumer advisory for animal foods served raw or under-cooked. Consumer advisory must include disclosure and reminder. Pf During inspection it was found that in a few spots on the breakfast and dinner menu that Fillet Mingon ( per employees it is offered under cooked) was no asterisked. Additionally on the dinner menu the disclosure and reminder were missing. These items must be corrected by 6/26/25 and a verification visit will be made to ensure correction.

During inspection a few dead pests were observed behind/around equipment. Upon questioning PIC stated that pest control tech had treated the facility recently and is due to be retreated in the next week. 6-202.15 - Outer Openings, Protected: Protect outer openings of establishment from insect or rodent entry (3) Solid, self-closing, tight-fitting doors. During inspection the rear kitchen door is heavily damaged with light coming through due to damage around the very loose door handle and at damaged sections along the door seal. Additionally the screen door is missing it handle and has gaps in the screening due to the tape being used to hold it to the door coming up. Rear door and screen door need to be replaced.

(Repeat) During inspection ice build up was found on sealed packages in the 2 door reach in freezer due a water leak in the rear of the cooling unit. Move items from under this leak until repair can be made and/or place a flat pan on the top shelf to catch ice build up. All items under affected by this leak are sealed with the exception of 1 bag of fries which was voluntarily discarded by the PIC.

During inspection 1 employee was observed towel drying dishes. CDI by having employee rewash and sanitize the observed dishes and allowing them to air dry.
47 1 Equipment, food & non-food contact surfaces approved, cleanable, properly designed, constructed & used No Yes No 4-501.11 - Good Repair and Proper Adjustment: Equipment shall be maintained in good repair. (Repeat) During inspection the following was observed: A handle on the main prep cooler is missing, a few of the prep tables have moderate-heavy rust build up on the lower shelves and legs, and the 2 door reach in cooler has a water leak. 4-205.10 - Food Equipment, Certification and Classification: Except for toasters, mixers, microwave ovens, water heaters, and hoods, food equipment shall be used in accordance with the manufacturer's intended use and certified or classified for sanitation by an ANSI-accredited certification program. During inspection 1 domestic reach in freezer was found in use as PIC states the commercial freezer went down and is not able to be repaired so the domestic cooler was brought in temporarily according to the PIC. Remove all non Ansi approved equipment.

During inspection the soda fountain was found with a water leak under the unit ( unclear if this is from the soda unit or the ice bin portion) resulting in water build up in the rubber pan under the cooler in the wait station. Repair the leak.

(Repeat) Today 1 hole was noted where ceiling panels have seperated over 2 door reach in cooler (Employees attempted to repair put the patch failed). Heavy floor damage observed from what appears to be water damage in the hall leading to the walk in cooler( the flooring has been replaced by decking boards however these boards are not sealed nor secured down). A few spots in the floor are damaged and need to be filled/ repaired by the kitchen entrance door and and some damage is noted by the 2 prep sinks by the grill that will need to be repaired. Repair building damage. 6-501.12 - Cleaning, Frequency and Restrictions: All physical facilities shall be maintained in good repair and shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. (Repeat) Additional cleaning need on the ceiling and corners of the floor along base boards due to grease splatter next to the fryers, and above the prep tables / reach in coolers due to food splatter.

During inspection less than the required lighting was found in the dish room, over dry/reach in storage, and the walk in cooler.
