Utensils shall be cleaned throughout the day at least every 4 hours if used with TCS (time/temperature control for safety) food. (CDI - Education was provided on when utensils should be cleaned and sanitized.)

Table mounted can opener blade needs to be replaced and replaced as often as necessary to ensure the blade is sharp. 4-202.16 Nonfood-Contact Surfaces - C -- Observed the faceplate missing pizza prep. Nonfood contact surfaces shall be free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance. Replace. 4-501.11 Good Repair and Proper Adjustment-Equipment - C -- Observed walk-in cooler door not self closing. Equipment shall be maintained in good repair and components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.

More frequent and deep cleaning is needed. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris and be cleaned at a frequency necessary to preclude accumulation of soil residues.

Sinks need to be cleaned more frequently to prevent build-up. Plumbing fixtures such as handwashing sinks, toilets, and urinals shall be cleaned as often as necessary to keep them clean. 6-501.19 Closing Toilet Room Doors - C -- During the inspection, the restroom door was opened. Except during periods of cleaning and maintenance, restroom doors shall be kept closed.

Place trashcan at handwashing sink. If disposable towels are used at handwashing lavatories, a waste receptacle shall be located at each lavatory. 5-501.115 Maintaining Refuse Areas and Enclosures - C - There is an accumulation of broken/unused equipment sitting in the corner of the store. A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items, and clean. Remove the equipment.
55 1 Physical facilities installed, maintained & clean No No No 6-501.11 Repairing-Premises, Structures, Attachments, and Fixtures-Methods - C - Floors, baseboards, and walls are damaged throughout kitchen and need to be repaired. Vent above heat cabinet is coming out of the ceiling and needs to be repaired. Physical facilities shall be maintained in good repair. 6-501.12 Cleaning, Frequency and Restrictions - C - Deep cleaning is needed on the floors, walls, and ceilings. Clean ceiling and walls behind equipment. There are multiple locations that have food residue build-up and need detailed cleaning. Physical facilities shall be cleaned as often as necessary to keep them clean.

Lockers or other suitable facilities shall be used for the orderly storage of employee clothing and other possessions. 6-501.14 Cleaning Ventilation Systems, Nuisance and Discharge Prohibition - C - Dust build-up has occurred on and around vents throughout the kitchen. Observed dust build up on the fan by the pizza package station, pizza oven and heat cabinets as well.General cleaning is needed on and around vents, outside of food preparation times to avoid contamination. Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other materials.
