NOTE: No points deduction: #4. Toilet fixtures shall be kept clean and in good repair. 15A NCAC 18A .2408 (d).- Observed in boy’s restrooms throughout the school, black plastic bag covered the urinals to prevent children from using it. EHS suggests to remove the plastic bag and post a sign “Do Not Use” instead. Keep all toilets clean and in good repair. Observed in Men’s restroom in the unused hallway. The toilet stall was broken, need repair. - PIC: Paul Barnhardt present during walkthrough inspection.
