Gravy was not under TPHC but had a 4 hour hold time on it for quality. The gravy hot hold unit appeared to be set below 125 F - Staff indicated that this was cooked shortly after 5 AM this morning. Maintain TCS foods in hot holding at 135F or above. PIC immediately discarded as there were only 10 minutes left on the 4 hour quality timer - reheated new gravy to 150 +, and placed back in unit to hot hold while the facility worked on the temp setting of the hot hold unit.

Observed box of happy meal boxes stored on floor tile in the back area of the store. Ensure that single-use items are stored in a clean and dry location, protected from contamination, and at least 6” off the floor. PIC moved off the floor at time of inspection.
