2-301.14 When to Wash (P)
Observed food employees handling raw chicken, cleaning, entering and exiting the walk-in cooler, and touching their faces without washing hands prior to changing or donning gloves. Additionally, front-line staff were observed reentering the kitchen after delivering orders outside without washing hands before resuming food handling duties.
Food employees shall wash hands at required times to prevent contamination and cross-contamination, including before donning gloves and when changing tasks.
Management immediately educated staff on proper handwashing procedures, and employees were observed washing hands more frequently. Corrected During Inspection (CDI)
2-301.15 Where to Wash (Pf)
Observed a cook rinsing his arms in the prep sink.
Food employees shall wash hands and arms only in designated handwashing sinks and may not use food preparation sinks for handwashing activities.
Management immediately instructed the cook to use only the designated handwashing sink. The cook immediately relocated to the handwashing sink and properly washed hands and arms. Corrected During Inspection (CDI). |

Metal food storage lids observed to be soiled. A stainless steel food storage container also observed to be soiled with batter and food debris.
EQUIPMENT FOOD-CONTACT SURFACES and UTENSILS shall be clean to sight and touch.

Beverage nozzles in the drive-thru and dining area were observed excessively soiled with buildup.
Equipment food-contact surfaces and utensils, including beverage dispensing nozzles, shall be cleaned at a frequency necessary to prevent the accumulation of soil or mold.
EHS informed management that the nozzles shall be removed, thoroughly cleaned, and sanitized, rather than only soaked in sanitizing solution. |

4-204.112 Temperature Measuring Devices - Functionality (C)
The interior of the walk-in cooler was observed missing an ambient air thermometer. An ambient air thermometer is recommended to be stored on the inside of refrigeration to verify proper unit operation and ensure accurate temperature monitoring.
In mechanically refrigerated food storage units, the temperature measuring device sensor shall be located to accurately measure air temperature in the warmest part of a refrigerated unit. |

A large hole was observed in the wall of the walk-in cooler. Additionally, damaged metal with exposed openings was observed on another wall within the walk-in cooler.

5-205.15 (B) System Maintained in Good Repair (C)
Plumbing beneath the three-compartment sink was observed leaking water. Additionally, the backflow preventer on the can wash faucet was loose and actively leaking. The men’s restroom handwashing sink faucet was also observed spraying water outside of the sink basin; a properly installed faucet screen should be installed to prevent splashing and ensure proper handwashing.
A plumbing system shall be properly maintained and kept in good repair. REPEAT |

Additionally, damaged wall tile located underneath the three-compartment sink requires replacement to ensure smooth, cleanable, and properly maintained wall surfaces. Lights observed to be blown underneath the hot holding unit.
