General CommentsDuring visit it was observed that 2 fryers and a flat top-griddle had been placed in kitchen on food preparation table after permit was written. Equipment is in use and is not under hood system. Equipment was not submitted for review or approved by the health department. Facility is not operating in accordance with approved plans. This is also a condition on the permit. No permits have been received by fire marshal as well. One door freezer that has been approved during permitting has been removed from kitchen, not working, and is stored behind facility. Please submit specifications sheets for one door freezer that is in kitchen currently. Food service equipment shall get prior written approval before bringing equipment into facility Fryers and flat top griddle was unplugged during visit. PIC has removed equipment from kitchen.
