General CommentsEHS authorized facility to use emergency TPHC. EHS sent a copy of the procedures to myerspark@cleanjuice.com. Facility printer was not working so EHS was unable to assist in completing procedures onsite. EHS explained procedures to PIC and how to complete the form. TPHC form must be completed and kept onsite. PIC was advised to contact EHS if they had questions about procedures or how to complete the form. EHS will make onsite visit within 3 days to determine if facility is following proper procedures. If facility is not following TPHC procedures during visit, EHS may issue an intent to suspend (ITS) for the facility permit. Facility has a 10 day VR for capacity to ensure they are capable of maintaining temperatures at 41F or below. Facility is not permitted to use TPHC as a long term solution to the lack of properly functioning cold holding equipment.
