5 30 LAVATORY FACILITIES: Fixtures clean and in good repair 30#; soap and individual towels or approved hand-drying devices provided 40# (.2409) No No No Repair two out of order handwash sinks in boys bathroom #1212. Both of these handwash sinks have broken hot water flow control valves beneath the sinks. Both of these sinks have extremely hot water flow that can not be adjusted due to the broken flow control valves. Priority repairs for safety are required due to the extreme hot water temps in these sinks being a potential scalding hazard. Repair one nonfunctional handwash sink due to a broken and loose faucet and a clogged drain in boys bathroom #2508. Repair one nonfunctional handwash sink due to a broken off water flow valve on the faucet in boys bathroom #2210. A total of four handwash sinks were out of order in student bathrooms at the time of inspection and in need of plumbing repairs.
6 30 FLOORS: WALLS: AND CEILINGS: Floors, walls, and ceilings clean and in good repair 30# (.2410) No No No Repair and stop the source of major ceiling leaks present in the shipping and receiving office #1308A and in the adjacent staff bathroom #1308B. These ceiling leaks appear to be adjacent to a ceiling sprinkler water supply pipe. Priority repairs are required due to large black stains being present on the drop ceiling panels in the leak locations. These stained ceiling panels should be removed and replaced after the sources of the leaks have been fixed. Replace missing ceiling panels in the gym and in the hallway at room #2305. Replace strips of lower wall base tile that have become loose and have missing sections on the school walls in the following locations: in the 5th grade hallway at the student bathrooms and drinking fountains, and on the second grade hallway on the half wall across from room #2105.
11 15 PREMISES: MISCELLANEOUS: Premises neat, clean 30#; no vector breeding or harborage 20#; pesticides and other toxic materials properly handled and stored 30* (.2415) No No No Remove the large stack of wooden pallets on the loading dock at the dumpsters. These wooden pallets should be removed from the school premises due to safety concerns that the stack of pallets could topple over and due to the potential for the stack of pallets to become a pest harborage area.
