4 20 TOILET FACILITIES: Walls and ceilings clean, nonabsorbent, washable, and in good repair 30#; fixtures clean and in good repair 40#; floors impervious, kept clean 30# (.2408) No No No Replace missing seat on one toilet in the boys bathroom on the #200 hallway by the cafeteria dining room. Repair two toilets that are nonfunctional due to poor water flow for flushing. One nonfunctional toilet is in the left side end stall in the girls bathroom on the #500 hallway and the other one nonfunctional toilet is in the girls bathroom on the #400 hallway by the elevator. Repair one nonfunctional toilet in the boys bathroom on the #500 hallway by classroom #503. A total of four toilets were observed to be out of order and in need of repairs at the time of inspection.
5 30 LAVATORY FACILITIES: Fixtures clean and in good repair 30#; soap and individual towels or approved hand-drying devices provided 40# (.2409) No No No Repair two hand wash sink drains that are clogged and backing in the girls bathroom on the #2500 hallway by the entrance of stairway #5. Repair one hand wash sink that has become partially detached from the wall behind it in girls bathroom #2408. Repair two hand wash sink faucets that have broken water flow control valves in the boys bathroom on the 2400 hallway by stairway #4. Repair two nonfunctional hand wash sinks in the boys bathroom on the #400 hallway by room #403. Replace broken plumbing drain pipework beneath the sink in art classroom #303. There are major leaks beneath this art class sink due to the broken pipework. A total of eight sinks were observed to require plumbing repair work at the time of inspection.
6 15 FLOORS: WALLS: AND CEILINGS: Floors, walls, and ceilings clean and in good repair 30# (.2410) No No No Replace broken and cracked window pane in stairway #4 at the landing between the ground floor and second floor. Repair and stop the sources of major roof and ceiling leaks in the following school locations: in art classroom #303, in classroom #2503, and on the #100 hallway at the entrance of the auditorium theater #109. Replace water stained and missing ceiling panels caused by the leaks in these locations after the sources of the leaks have been fixed. Replace broken door window frame in the special needs class kitchen #405. Repairs for safety are required due to the splintered wood on this broken door window frame.
8 40 LIGHTING AND VENTILATION: Fixtures, grills, vents, blinds, drapes, etc., clean and in good repair 40# (.2412) No No No Repair nonfunctional air conditioning systems in the following six school building classrooms: in classroom #413, in classroom #415, in classroom #417, in classroom #519, in classroom #521, and in classroom #523. Repair nonfunctional light fixtures in the staff bathroom on the #400 hallway by custodial closet #414. There was no functional lighting in this staff bathroom at the time of inspection. Priority repairs are required. Repair nonfunctional ceiling light fixtures in the entrance of the boys locker room, and in the boys bathroom on the #400 hallway by room #403.
