Replace water damaged and water stained ceiling panels in the listed ceiling leak locations after the sources of the ceiling leaks have been fixed. Repair and repaint water damaged lower wall surface caused by the roof leak around the skylight window on the top floor hallway of building (B). Repair clogged floor drain for washing machine in the cafeteria kitchen laundry room. Standing water is covering the floor in this laundry room. Priority repairs for safety are needed due to the wet laundry room floor being a slip and fall hazard and the power cord for the washing machine being in standing water which creates possible electrical hazards. Repair clogged floor drain in the coaches office/first aid room #1413 in the gym. This floor drain periodically backs up and overflows. There is a sewer gas odor coming from the shower and bathroom floor drains in the gym girls locker room bathroom. These three floor drains should be flushed out weekly with water by custodial staff. These floor drain pipes should also be cleared of any blockages and buildup by building maintenance. Remove and replace three broken glass mirror panels from the wall of the fitness classroom #1701 A. Priority repairs for safety are needed.
8 20 LIGHTING AND VENTILATION: Fixtures, grills, vents, blinds, drapes, etc., clean and in good repair 40# (.2412) No No No Repair poorly functioning air conditioning systems throughout all of the hallways, classrooms, offices, and bathrooms of building (H). The ceiling ventilation ductwork in the gym girls locker room and bathroom/shower room is covered with build up and residue stains from moisture condensation and humidity. The ceiling ventilation ductwork should be cleaned of these stains and should be repainted with a water proof and mold resistant paint after cleaning.
11 10 PREMISES: MISCELLANEOUS: Premises neat, clean 30#; no vector breeding or harborage 20#; pesticides and other toxic materials properly handled and stored 30* (.2415) No No No Cockroaches are present throughout buildings (A) and (H) and are being spotted frequently by staff. Pest control treatments are needed throughout both buildings to remove the presence of the roaches.
