Single use articles may be stored and handled to prevent contamination. Observed single-use cups being stored in housekeeping closet on shelf without any plastic sleeving. Cups were discarded.
21 1 Handwashing sinks supplied and accessible; toilet tissue supplied No No No 15A NCAC 18A .1826(a) Noted handsoap and paper towels missing from the laundry room handsink. Handwashing sinks as required in Rule .1825 in this section shall be supplied with hand soap, and either individual, disposable towels; a continuous towel system that supplies the user with a clean towel; a heated-air hand drying device; or a hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures. CDI- EHS instructed PIC to replace missing hand soap and paper towels in noted area.
22 2 Water from approved source; backflow prevention; plumbing in good repair No No No 5-205.15 Maintain a plumbing system in good repair. Observed both lobby restrooms out of order. The first restroom had no water in the toilet. The second was not evaluated due to the manager not having access.
26 4 Furnishings clean and in good repair; guest room fixtures clean and sanitized between guests No No No 15A NCAC 18A .1826 (h) - Furnishings, bathroom fixtures, carpets, and other accessories in guest rooms, shall be kept clean and in good repair. Noted chairs are worn or damaged in the following guest room: 101, 122, 319 and 322, 324,333, 236 Observed damage to bed frame in rooms: 329, 324 Observed soiled mattresses the following guest rooms: 414, 429, 327,323, 324, 333, 334, 225, 215, 206, 109,117 Observed water damage in dresser and nightstand drawers in all viewed rooms. Observed soiled bathtubs and bath mats in the following guest rooms: 414, 429, 329, 333,236, 229, 225, 206, 106, 109, 119 Observed mold on the back panel of the nightstand in guest room 106. Observed damage to bathtub in room: 323, 334 Observed soiled shower heads in all viewed rooms.

Observed dust accumulation in all of the bathroom ceiling vents that were viewed. Please remove cover to access the internal area. Observed accumulation and damage to the majority of the HVAC unit throughout the establishment including the units in the hallways. Observed wall damage in rooms: 412, 324, 106, 117, behind ice machine on 1st floor. Observed baseboards not connected to walls in all guest areas viewed. 28 4 Insects and rodents present; live animals prohibited from food preparation, storage, sales, display, or dining No No No 15A NCAC 18A .1826 (i) Maintain premises free of insects, rodents, and other pests. Observed multiple live and dead cockroaches in multiple guestrooms. Roaches observed in rooms: 412, 414, 429, 329, 327, 323, 324, 334, 229, 106, 109, 119, 117.
30 4 Linen clean and in good repair; two sheets provided for each bed No No No 15A NCAC 18A .1828 (a) Clean bed and bath linen, in good repair, should be provided for each guest. Observed soiled linens in the majority of the rooms viewed.
31 1.50 Linen properly handled and stored No No No 15A NCAC 18A .1828 (e ) Carts used for soiled laundry shall be labeled or identified for soiled laundry use only. Observed laundry cart labeled as clean but contained soiled linens.
33 1 Garbage and refuse disposal; facilities maintained No No No 5-501.115; Observed multiple wooden pallets and metal barrels in the dumpster area. A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean. 15A NCAC 18A .1825( e) All refuse shall be collected and stored in covered receptacles. Refuse receptacles shall be kept clean and in good repair. Observed open doors on dumpster.
34 3 Premises maintained to prevent breeding and harborages No No No 15A NCAC 18A .1826(i) The premises and guest rooms shall be maintained free of insects, rodents, and other pests. The presence of insects, rodents, and other pests shall be controlled to eliminate their presence on the premises and eliminating harborage conditions. Observed cockroaches in 13 out of the 18 rooms inspected. Floor three is currently closed to guest due to pest activity. The rooms have not been cleaned. They planned to clean after the treatment. However, the vacant rooms with trash, left over food, soiled towels and linens are creating a harborage for pest.
37 1.50 Household cleaning agents, medicines, and sanitizers properly stored and handled No No No 15A NCAC 18A .1827( c) Household cleaning agents such as bleaches, detergents, and polishes shall be used and stored according to manufacturer's recommendations. On the housekeeping cart, there was no sanitizer available to properly clean restrooms. EHS spoke with PIC and took sanitizer to the housekeeper on duty.
