Pf ***The can opener had residue on it. Multiple bins on the clean dish rack had food particles on them. The drink nozzles on the drink machine at the drive thru and dining area had build up. Employee set pan on the ground while cleaning, and then set the pan on a tray that was on a prep table. CDI, can opener, pan, trays and drink nozzles were placed at the 3-compartment sink to be washed, rinsed and sanitized. |

Multiple ceiling tiles and floor tiles are in poor repair/ broken. Multiple of the floor drains have build up residue. Floor drain has been tiled over due to a pipe not working. Observed multiple areas in the kitchen that water is sitting on the floor due to drainage issues. |
