Employee was observed washing hands sometimes when appropriate but not all the time. CDI- Employee wash hands and returned to work.

Raw beef roast was stored above RTE product in the walk in cooler. CDI- Roast was moved to the bottom shelf. RTE food packaging did not appear to be contaminated. It was intact and dry.

Inside components of ice bin had an accumulation of soil and/or mold at the top dispensing plate. - PIC contacted company to have ice machine disassembled and deep cleaned.

PIC stated it falls when hung the correct way with mop head down. Spoke with PIC about having maintenance drill a hole in handle and providing a way to hang in to prevent contamination. 6-501.12 Cleaning, Frequency and Restrictions (C): Physical facilities shall be kept clean. Walls, floors, ceilings and vents throughout kitchen needs additional cleaning. Areas include but not limited to under dish machine, and under prep sink. 6-501.11 Repairing - Premises, Structures, Attachments, and Fixtures - Methods (C): Physical facilities shall be kept in good repair. Coving tiles are cracked and/or detached in several areas throughout the kitchen.
