6-301.12 Hand Drying Provision (Pf)- Observed the paper towel dispenser in the employee restroom was empty. Staff member that was exiting the restroom as the restroom was being checked used the last of the paper towels. CDI- Staff refilled the paper towels. All handwashing sinks shall be supplied with approved hand-drying provisions. |

4-602.11 Equipment Food-Contact Surfaces and Utensils - Frequency (C)- Observed the ice bin had visible organic growth in the bottom of the ice bin. CDI- Staff denatured the ice immediately by putting soap into the ice and staff began cleaning the ice bin shortly after. Food-contact equipment such as ice bins shall be cleaned at a frequency to keep them clean. |

4-501.11 Good Repair and Proper Adjustment - Equipment (C)- Observed ice build-up in the walk-in freezer on the condenser. Ice build-up was also observed at the floor along the door seal from a possible air leak. Needs to be repaired. Equipment shall be maintained in good repair. |
