General Comments- Facility has undergone a name change and change of concept since the previous inspection (documented in previous visit sheets). EHS confirmed that the facilities LLC remained the same and that it is still listed as active on the Secretary of State website. Owners stayed the same, there is just a new management crew on site to run day to day operations. Changes remain cosmetic in the dining room areas with the kitchen remaining untouched with no addition or removal of equipment. Facility currently is only offering a small menu of a few grilled cheese options containing only RTE ingredients. PIC stated that they have plans of changing the menu in the future and was informed to contact the EHS prior to making changes to the menu. Facility is well equipped to serve the current contents of their menu.
