4-501.11 Maintain equipment in good repair. Observed a gasket that has an air gap for one of the thaw cabinets. PIC stated they are ordering a new door for the unit. It is able to maintain temperature at this time. Observed a dishmachine that is down and locked out. PIC stated they are using 3 compartment sink to sanitize dishes at this time and they have reached out to HOBART about getting the machine repaired/replaced. REPEAT |

6-501.11 Floors, walls, and ceilings including the attachments shall be maintained in good repair. Observed areas of the kitchen with broken floor or coving tiles. PIC showed the REHS that they placed a work order on 5/28/26 and followed up on 6/8/26. They will reach out again to get the floors repaired. REPEAT |
