PIC stated he has it but when cleaning it the certificate was knocked off the wall and into a mop bucket. PIC could not provide a copy using website. PIC shall be a Certified Food Protection Manager who has shown proficiency of required information through passing a test that is part of an accredited program. Manager stated he is still waiting to receive a copy of his certificate. Obtain copy of certificate as soon as possible.
16 1.50 Food-contact surfaces: cleaned & sanitized No No No 4-501.114 - Mechanical warewashing machine was observed not dispensing proper amount of chemical sanitizer. Warewashing machine must dispense proper amount of chemical sanitizer that matches the temperature at which the machine operates. At time of inspection, no chemical sanitizer was observed on test strips and there was no strong smell of chlorine. Additionally, upon trying to prime machine, there was no evidence that machine was dispensing sanitizer. Have machine serviced- until proper amount of sanitizer is being dispensed, employees have been instructed to use the 3-Bay to wash, rinse and sanitize equipment and utensils.

Sauces exceeded the date in which their date marking labeled has listed as their use by date. Items shall be discarded or used by the date listed on labels and not held over a max of 7days. Items were properly labeled. CDI - manager voluntarily discarded sauces. Ensure all items are being discarded or use by within proper time frames.
25 0 Consumer advisory provided for raw/ undercooked foods No No No 3-603.11 (B)(2) Consumer Advisory Disclosures that are asterisked to a footnote shall state, * Items are served raw or undercooked, or contain (or may contain) raw or undercooked ingredients. -Pf - Upon review of the menu and discussion between employee, it was determined that the T-Bone steak listed on the menu is sometimes cooked to order (per the customer); all other foods are cooked well done. The T-Bone steak on the menu was not observed being asterisked to refer to the footnote and reminder . Items that are to be cooked to order must be identified by asterisking them to a footnote that states that items are served raw or undercooked or contain (or may contain) raw or undercooked ingredients. Ensure T-Bone steak is asterisked on each menu.

Ensure toxic substances and other poisonous materials are stored separately and properly. CDI - manager moved pump sprayer and ensured that items below were not contaminated.

Ensure items are either loosely covered or uncovered if protected from overhead contamination.
