Discussed these items withe the employees, who restructured the order to put raw foods on the freezer side with other raw foods. Keep raw animal foods stored below cooked and ready-to-eat foods. (A)(4) Provide lids for the fruits in the freezer in the Rush Bowls area. All of the foods here are uncovered.

Ladle was taken to the 3-compartment sink to wash. Wash and sanitize all utensils at least every four hours after they have been used. 4-602.12 Cooking and Baking Equipment (C) Clean the food debris from the oven interior in Most Thai area. 4-703.11 Hot Water and Chemical - Methods (P) Utensils were placed in a sanitizer solution and removed to air dry that employee stated had chlorine sanitizer in it, but the sanitizer measured 0 ppm. Water was removed and repoured. Sanitizer products shall be mixed as required for sanitizing utensils.

3-501.17 Ready-To-Eat Time / Temperature Control for Safety Food, Date Marking (Pf) In the Most Thai area, in Avantco cold prep unit, cooked rice containers were declared as cooked on 4/27/2026, but the employee labeled them with date 4/28. Also, cooked and frozen curry containers in the M-B freezer did not have a label on them, and employee labeled them with a date of 4/28. However, employee stated they were cooked on 4/27/2026. All were relabeled. In the Rush Bowls area, in the Empera cooler, oat milk, milk, and soy milk did not have labels on them, and employee had no idea when the containers were opened. All were discarded. If TCS foods are cooked or opened from a commercial container, or are cooked and cooled, and are maintained in the establishment for more than 24 hours, then they shall be labeled with the date of preparation or date to discard, and the label shall be removed and replaced with an updated label whenever new TCS foods are added.

Provide a more durable finish than the duct tape to make a smooth, easily cleanable surface. Repair the holes in the wall to the right of the back door behind the fire extinguisher. 6-101.11 Surface Characteristics - Indoor Areas - The side area next to the cooking station is now used for cooler storage and spice storage, as well as cooling foods, and the hot box. Replace the absorbent ceiling tiles with nonabsorbent ceiling tiles. Replace the cardboard sitting on the partial wall to the right of the stove. Since the previous inspection, the metal corner piece that was added needs to be filled in and made smooth and easily cleanable, as it has many holes and exposed screw heads. Materials shall be nonabsorbent and easily cleanable. Seal the wooden support beam for the Rush Bowls hand sink to make it nonabsorbent, smooth, and easily cleanable. 6-501.12 Cleaning, Frequency and Restrictions (C) Clean the floor under the side storage room equipment. 6-501.113 Storing Maintenance Tools (C) On the utensil rack, store the floor fan and extension cord in a location away from the utensils.
