General Comments

Note: An ice machine was added in the nurse’s room (left of front desk) after CO. Ice machine not indirectly drained but tied into a pump which pushes the water to the sink discharge plumbing. Staff said the breaker keeps tripping and then there is water on the floor. I observed water pouring onto the floor from the pump, did not hear pump operating. Staff turned off ice machine until problem is fixed. Note: vinyl wrapped ceiling tiles missed on approved plans
