3-501.17 Ready-To-Eat Time / Temperature Control for Safety Food, Date Marking (Pf)-REPEAT- Cooked mushrooms and chicken were noted in the grill drawers with no date mark and person in charge stated they were cooked yesterday. American cheese and hot dogs that were opened yesterday was observed with no date mark. Salsa cups in the taco station cooler were made yesterday and they were not dated. Pulled chicken in the Walk in Cooler had a date of 4/8, person in charge stated it was frozen on the 8th and thawed yesterday. (A) Refrigerated, READY-TO-EAT, TIME/TEMPERATURE CONTROL FOR SAFETY FOOD prepared and held in a FOOD ESTABLISHMENT as well as READY-TO-EAT TIME/TEMPERATURE CONTROL FOR SAFETY FOOD prepared and PACKAGED by a FOOD PROCESSING PLANT that has been opened and held for more than 24 hours shall be clearly marked to indicate the date or day by which the FOOD shall be consumed on the PREMISES, sold, or discarded when held at a temperature of 41F or less for a maximum of 7 days and may not exceed the manufacturer’s use-by date. When freezing cooked items the cook date, freeze date, and thaw date shall be provided. (PF) CDI- Person in charge dated the food items. |

4-501.11 Good Repair and Proper Adjustment - Equipment (C) REPEAT- The gasket on the outdoor Walk in Cooler/Freezer combo is torn. Rusted shelved noted in the walk in cooler and the grates of the soda fountains were rusted. Cabinetry at the POS area is damaged and the laminate is peeling off. Icecream sandwich cooler had an ambient of 53.8F today, not TCS items were stored in it. Equipment shall be maintained in good repair.

6-501.11 Repairing - Premises, Structures, Attachments, and Fixtures - Methods (C)-REPEAT- Noted a few small holes in the wall by the dish machine that need filled in. There is a leak from the ceiling in the women's restroom and water damaged ceiling tiles. Damaged ceiling tile noted on the cookline. Floors, walls, and ceilings including the attachments such as soap and towel dispensers; light fixtures; and heat/ac vents shall be maintained in good repair.
6-501.12 Cleaning, Frequency and Restrictions (C)-REPEAT- Cleaning needed of the wall behind the dish machine and of ceiling vent covers throughout the kitchen. Stained/soiled ceiling tiles noted above the dough cooler. All physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
6-201.11 Floors, Walls and Ceilings - Cleanability (C)- Noted acoustic/absorbent ceiling tiles in the women's and men's restrooms and both restrooms have hose bibs. Establishment must provide floors, floor coverings, walls, wall coverings, and ceilings that are designed, constructed and installed so they are smooth and easily cleanable. Antislip floor coverings may be used for safety reasons. |
