General CommentsGentle reminder: doing a great job at the dish washing area with designated employees dirty to clean. One cook i did not observe changing tasks to require handwashing. Just remind staff when they leave an area that is a task change. Handwashing is always a preventive to illness in establishment. Mop utility room has been in poor condition since last inspection. Dishmachine has been leaking since last inspection. There was a roach noted last inspection in June. These areas need immediate attention. The mop utility room may be the source of the insect too. Wall is severely damaged and it is a wet area. Need someone to on duty with a certificate as a Certified Food Protection manager.

PIC stated mop water is sometimes dumped outside. Repair mop sink to function as intended. This is a repeat.
