EHS observed a dead roach on the electrical box behind the stand up fridge. EHS observed dead roaches underneath the food preparation sink. EHS observed many dead roaches in the light shield cover above the food prep sink. Discard dead pests when observed and clean surfaces where they are found.
6-501.111 Controlling Pests - EHS observed numerous flies in the kitchen and dining area. Take steps with your certified pest operator to remove pest presence. |

4-205.10 Food Equipment, Certification and Classification (C) - EHS observed a recently purchased blender on the equipment storage rack that stated "house hold use only" on the bottom. EHS observed what looked like a non commercial meat grinder above it. EHS could not verify the grinder was commercial or ANSI approved based on the packaging and equipment data plate. Besides microwaves, mixers, toasters, hood systems and water heaters, equipment in the kitchen must be for commercial use and ANSI approved. Remove or replace unapproved equipment. |

PIC stated it can take up to 15 minutes sometimes for the lights to turn on once the light switch is flipped. Repair lights in the storage room to provide the required foot candles of light whenever the light switch is turned on. |
