Observed cooked carrots and stew beef with rice in a plate that was setting on the counter top at room temperature that was below 135F. PIC gave plate to client. Educated staff that they have to keep TCS foods that they serve to the clients that was received hot has to be kept at 135F and above. Also advised for staff to only make plates for the number of clients at facility and if any other clients come in to the make them plates. These way staff can keep food in stove/hot holding unit at 135F and above.
