NO POINTS FOR THE FOLLOWING VIOLATION: #6. Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410. Observed minor stained ceiling tile near boys restroom located near stairwell on level 2. EHS advises facility to ensure no active leak is occurring and to have stained ceiling tile replaced. #7. All storage shall be at least 15 inches above the floor or otherwise arranged so as to permit thorough cleaning. 15A NCAC 18A .2411. Observed boxes of storage on the floor in storage closet located on second floor. *NOTE: Operations Manager, Annie Abraham, was present during time of inspection.
