3-603.11 (A) Provide consumer advisory for animal foods served raw or under-cooked. Consumer advisory must include disclosure and reminder.-Pf: Mushroom toast containing egg yolk not covered under Consumer Advisory. Remove asterisks on menu items that do not contain raw or under-cooked protein ingredients ie bread pudding, crme brlée, Korean street corn. Verification within 10 days. Send inspector photo or file of corrected menu.
