2-102.11 - Demonstration (A-C) (1-17): Observed during the inspection that the PIC was not a Certified Food Protection Manager, observed priority and priority foundation violations, and the PIC was unable to answer food safety questions outlined and listed in section C of this code. Based on the RISKS inherent to the FOOD operation, during inspections and upon request the PERSON IN CHARGE shall demonstrate to the REGULATORY AUTHORITY knowledge of foodborne disease prevention, application of the HAZARD Analysis and CRITICAL CONTROL POINT principles, and the requirements of this Code. The PERSON IN CHARGE shall demonstrate this knowledge by: (A) Complying with this Code by having no violations of PRIORITY ITEMS during the current inspection; or (B) Being a certified FOOD protection manager who has shown proficiency of required information through passing a test that is part of an ACCREDITED PROGRAM; or (C) Responding correctly to the inspector's questions as they relate to the specific FOOD operation. |

2-501.11 Clean-up of vomiting and diarrheal events Pf: Observed no clean-up policy. A food establishment shall have written procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter. CDI the REHS provided a copy of the clean-up procedure and reviewed the policy with the PIC. |

5-205.11 - Using a Handwashing Sink Pf: Observed the hand washing sink located in the back room was not turned on and water does not flow out of the faucet, employees are not able to use this sink to wash their hands. Maintain access to hand sinks at all times. A Verification is required within 10 days.

A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks. CDI sings were provided by REHS at all three hand washing sinks. |

4-601.11(A) - Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils Pf: Observed several soiled pizza pans sitting on the oven ready for use. Equipment food contact surfaces and utensils shall be clean to sight and touch. REHS showed the PIC and one pan was replaced with a new one due to charred build up that could not be removed. CDI pans were washed for a correction.
4-702.11 - Before Use After Cleaning P: Observed when asking the PIC that surfaces and utensils are not being sanitized. Utensils and Food contact surfaces shall be sanitized before use after cleaning. PIC could not explain how to wash dishes properly and did not have sanitizer to show. REHS explained sanitizing requirement and spoke with the owner on the phone. REHS will be scheduling an education visit and verification with the owner and all staff that need training. |

4-205.10 - Food Equipment, Certification and Classification: Observed an unapproved mini refrigerator being used to hold TCS foods for the pizzas. Except for toasters, mixers, microwave ovens, water heaters, and hoods, food equipment shall be used in accordance with the manufacturer's intended use and certified or classified for sanitation by an ANSI-accredited certification program. The fridge MUST be removed and was not approved when the permit was issued. Based on the transitional permit issued a flip top make station was provided then removed. |

4-302.14 - Sanitizing Solutions, Testing Devices: Observed sanitizer tablets that require QUAT test strips which were not provided or available. A test kit or other device that accurately measures the concentration in mg/L (or ppm) of sanitizing solutions shall be provided. A verification may be required if the tablets are used. According to the owner they are not used.
4-303.11 Cleaning Agents and Sanitizers, Availability (A) Pf: Observed sanitizer that is used was not available. Cleaning agents that are used to clean EQUIPMENT and UTENSILS as specified under Part 4-6, shall be provided and available for use during all hours of operation. Bleach test strips are provided and the owner stated that bleach or "high low sanitizer" brand was used but this was not in the store. Owner was not able to determine the sanitizer used and the PIC that was present did not know what the sanitizer was or how to make it. A 10 day verification is required and a training will be scheduled with the owner. The quat tablets can be used if they function properly but there is no way to test the sanitizer. |

5-501.13 Receptacles (A): Observed a cardboard box with a plastic bag liner used as the trash receptacle. Except as specified in (B) of this section, receptacles and waste handling units for REFUSE, recyclables, and returnables and for use with materials containing FOOD residue shall be durable, cleanable, insect- and rodent-resistant, leakproof, and nonabsorbent. The section B allows for the plastic liner but the cardboard trash can is not allowed. There is no exemption for trash receptacles not being made of nonabsorbent material. |

6-201.11 - Floors, Walls, and Ceilings: Observed damaged floor tiles in the back Warewashing and storage room. Establishment must provide floors, floor coverings, walls, wall coverings, and ceilings that are designed, constructed and installed so they are smooth and easily cleanable. Repair flooring and provide tiles in areas that are missing. Also, observed the large deep freezer located at the front is sitting on original packaging of Styrofoam and cardboard. This material needs to be removed and the freezer needs to sit on the approved tile flooring.
6-101.11 - Surface Characteristics: Observed acoustic foam style ceiling tiles over the deep freezer and hand washing sink area and observed exposed wood on the wall behind the deep freezer. Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. Cover the wood with approved FRP and replace the ceiling tiles with approved coated tiles that are in the rest of the food service area. |
