There were cleaned utensils observed stored in bins with clean rags at the bottoms and other bins with crumbs and debris at the bottoms. Cleaned EQUIPMENT and UTENSILS, laundered LINENS, and SINGLE-SERVICE and SINGLE-USE ARTICLES shall be stored in a clean, dry location where they are not exposed to splash, dust, or other contamination and at least 6 inches above the floor. CDI, the PIC put the items in the bins in the sink to be washed. |

Observed the door on the front reachin cooler damaged and some rusty buildup on the racks inside. The door closer on the walkin cooler door was missing. Equipment shall be maintained in good repair. Replace the door closer and repair the front cooler door. |
