No points deducted for the following: 2: Keep all flooring clean. 15A NCAC 18A .1309 (a). Observed stained carpeting in resident room E204 and A208. Observed carpet staining in both occupied and unoccupied resident rooms. Carpeting was also stained in main hallway in the D wing and main hallway near entrance. 16. Observed a partial backup water supply plan. Provide a completed one. Template emailed to Renee Walburn. 28: Suitable rooms or spaces shall be provided for the storage of all necessary equipment, furniture, and supplies, and kept clean. 15A NCAC 18A .1318 (a). Observed multiple bags stored on the ground in biohazard room on the 2nd floor. 32: Mattresses shall be kept clean, dry, and odor free. 15A NCAC 18A .1319 (a). Observed stained mattresses in room B205. 35: Patient contact items shall be kept clean and in good repair. 15A NCAC 18A .1319 (c). Observed patient single service items stored on the floor in wellness room on 1st floor. *Inspection conducted by Sam Cary.

Observed the downstairs bistro area and room D105 water also registering above 116F.
