No points were deducted for the following: 33. Observed staining on sheet in room 4021. Staff informed me that resident makes their own bed and that this resident in particular was moving out this room currently. Ensure that linen is clean even on beds that residents make themselves. **Clean bed linen in good repair shall be provided for each individual and shall be changed when soiled. 15A NCAC 18A .1319 (b). NOTE: Juan Esquivel was present during the inspection.
