Inspection conducted with Kevin Smith. Due to COVID-19 precautions, inspection was not signed by the artist. Artist using all disposable equipment. Autoclave not required. Sink in restroom for artist use only. Door to remain open when tattooing. Permit issued. “Executive Order No. 141 or subsequent Orders are in effect unless such Orders are rescinded or superseded by another applicable Order.”
