14 1.50 Food-contact surfaces: cleaned and sanitized Yes No No 4-501.114 Manual and Mechanical Warewashing Equipment, Chemical Sanitization-Temperature, pH, Concentration and Hardness - P. Ensure dish machine final rinse water measures free chlorine at 50-200 ppm. Observed dish machine final rinse water measuring 0-10 ppm free chlorine. CDI - PIC changed out bottle of concentrate and set up machine again. After correction, final rinse water measured 100 ppm free chlorine. 4-601.11(A) Equipment food contact surfaces and utensils shall be clean to sight and touch. -Pf. Observed soiled slicer stored as clean. Food debris remained on back surface of blade. CDI - PIC removed for re-cleaning.
20 3 Proper cold holding temperatures No Yes Yes 3-501.16(A)(2) Maintain TCS foods in cold holding at 41F or less. -P. Observed several areas where TCS foods were stored and measuring above 41.0F. See temperature chart. Some issues were due to stocking food into line coolers before food had properly cooled to 41.0F. Other issues may have been due to cooler drawers or doors being left partially opened or being opened frequently. PIC said that food had been stocked after noon today. Today the PIC Corrected by removing hamburger patties, sliced tomatoes, cut lettuce, and hot brined pickles from line coolers and placing into the freezer to rapidly complete cooling; however, the veggie burger mix was voluntarily discarded as it was in a large container and stored in an undercounter cooler that was not holding foods at or below 41.0F and it had been there for 6 days. No other TCS foods were in that cooler at the time. VR - verification required.
23 0.50 Consumer advisory provided for raw or undercooked foods No No Yes 3-603.11 Provide consumer advisory for animal foods served raw or under-cooked. Consumer advisory must include disclosure and reminder.-Pf. Observed facility offers undercooked shell eggs. Menu board on the wall had no consumer advisory. Printed menu on the counter had a disclosure asterisk on the egg menu item and a reminder at the bottom of the menu but it lacked a disclosure footnote. Explained corrective actions needed to the PIC. VR - verification required.
31 0.50 Proper cooling methods used; adequate equipment for temperature control No No Yes 3-501.15 Quickly cool foods. Use methods such as open/vented shallow pans, large ice baths and active stirring. Cold air must flow around product to remove the heat. -Pf 4-301.11 Provide equipment in number and capacity so that cooling, heating, and holding temperatures are achieved. -Pf Observed facility is having cooling issues due in part to the methods used and also due in part to inadequate equipment capacity for the amount of food in house. Facility has no walk in cooler. Discussed with PIC. She said a truck had come today and that they receive deliveries three days a week. Inspector and PIC spoke with employees who had pattied out the burgers, sliced the tomatoes, and cut the lettuce. Discovered that items went straight from prep to line coolers, covered, and without any re-chilling step in between. Today corrections were made by placing loosely covered pans of these items into the reach in freezer, starting with those closest to the 4 hour cooling limit. Cut lettuce, sliced tomatoes and pattied burgers cooled rapidly and were approaching 41.0F when inspection ended. VR - verification required.
36 1 Insects and rodents not present; no unauthorized animals No No No 6-501.111 Keep the premises free of insects, rodents, and other pests. Observed several house flies in kitchen.
45 0 Equipment, food and non-food-contact surfaces approved; cleanable, properly designed, constructed and used No No No 4-501.11 Maintain equipment in good repair. 52 0 Garbage and refuse properly disposed; facilities maintained No No No 5-501.114 Ensure drain plug on dumpsters, waste containers is in place. Observed dumpster is missing the drain plug.
