General CommentsInspection conducted by Zach Holtsclaw under guidance of EHS Paul Miller REHS #3468. EHS is also going to provide new handwashing signs due to the wear down of the current handwashing signs.

EHS also observed sticker residue on trays. CDI: PIC voluntarily rewashed and removed sticker residue.

Guiding EHS observed several plastic dishes with melted portions or cracks in them. EHS showed this to PIC and she voluntarily discarded them. CDI.
49 0 Non-food contact surfaces clean No No No 4-601.11 (B) and (C) Equipment, Food-Contact Surfaces, Nonfood Contact Surfaces, and Utensils (C) (C) NonFOOD-CONTACT SURFACES of EQUIPMENT shall be kept free of an accumulation of dust, dirt, FOOD residue, and other debris. Guiding EHS noticed slight dust and debris accumulated on a scale.
55 0 Physical facilities installed, maintained & clean No No No 6-501.12 Cleaning, Frequency and Restrictions (C) (A) PHYSICAL FACILITIES shall be cleaned as often as necessary to keep them clean. EHS observed debris buildup in the cooler ceiling. 6-501.114 Maintaining Premises, Unnecessary Items and Litter (C) The PREMISES shall be free of: (A) Items that are unnecessary to the operation or maintenance of the establishment such as EQUIPMENT that is nonfunctional or no longer used. Guiding EHS observed a commercial mixer which is no longer in use. It shall be removed as soon as possible. Especially since it has debris and dust accumulated on it. 6-501.11 Repairing - Premises, Structures, Attachments, and Fixtures - Methods (C) PHYSICAL FACILITIES shall be maintained in good repair. Guiding EHS noticed a leaking soap dispenser in the restroom. There is also a damaged floor drain in the kitchen. The leaking soap dispenser could get soap on the faucet handles and then food employees could get soap on their hands after turning off water; thus contaminating gloves and/or food with soap.
