Dish machine was testing 0ppm chlorine. Facility was washing dishes in the dish machine upon arrival. CDI: Chemical bottle replaced. PIC did not prime machine, so items were still not properly sanitized. PIC was shown how to prime machine and shown why if it is not primed, there is no sanitizer in the water. Machine was primed and then machine tested at 100ppm chlorine. 4-602.11 Clean the equipment and utensils used with nonTCS foods as required to avoid contamination. Observed black and pink build up on inside of ice bin in dining area.

CDI: Noodle containers opened and then food cooled at a rate of .33F/min. Observed noodles cooling in the same way as previously stated in downstairs kitchen, but at a rate of 0.10 F/min. Tryon st location yesterday dated 12/11. CDI: Proper date placed on product. Observed broth dated 12/5 in reach in that was frozen. Employee stated that product was frozen at N. Tyron st location. CDI: Pulled from frozen (PFF) date marked on broth and explanation given to employee that the date that food is taken from frozen to thaw, should be added to the original sticker. REPEAT VIOLATION

Observed hot items labeled for discard at 2:30, not discarded until Environmental Health Specialist (EHS) directed PIC to discard. Observed cold items labeled for discard at 3pm still in cooler at 3:30pm. CDI: EHS directed PIC to discard items as it has exceeded 4 hours. Observed no time written on noodles in main floor kitchen. CDI: Correct time written. Observed tapioca pearls pulled at 8am still on counter at 1:30pm. CDI: Product voluntarily discarded. REPEAT VIOLATION

Observed damaged walls, ceiling tiles with heavy discoloration or missing, a leak in the downstairs back area from the ceiling, and areas throughout kitchen that are not smooth and easily cleanable. All items except leak are part of transitional permit. Observed tiles broken and floor "sinking" with weight near main floor closet and in front of double door reach in on main floor. Observed water accumulation under tiles and onto floor in main floor closet. Observed water stained ceiling tiles. REPEAT VIOLATION 6-501.114 Discard items and litter stored on the premises as they are not necessary to operation and maintenance. Observed upstairs closet with excessive amount of items stored on floor that are not part of normal operations.
