No points were taken for the following: 3. Observed molding torn in room 612. Needs to be repaired. **Keep all walls and ceilings clean and in good repair. 15A NCAC 18A .1310. 8. Observed stained toilet seat in room 216. Needs to be cleaned. Only one seat stained. **Keep all toilet equipment clean and in good repair. 15A NCAC 18A .1312 (b). 10. Observed bed pans and urinals not labeled in rooms 113, 104, 216 and 205. Ensure that bed pans and urinals are being labeled. **Label all disposable bed pans with the residents/patients name and date. 15A NCAC 18A .1312 (c). 27. Observed several pet records expired. If animals are no longer coming into facility, remove old records that are expired. If animals are still coming into facility, records needs to be updated. **Maintain adequate vet records for all animals. 15A NCAC 18A .1317 (f). 34. Observed bags of clean linen stored on the floor in the downstairs linen storage. **Clean linen shall be stored and handled in a separate room or area, or in another manner that will prevent contamination of clean linen. 15A NCAC 18A .1319 (b). 44. Observed box of Ensure stored directly on the floor in Med Prep room on Midwood hall. **Containers of food shall be stored above the floor, on clean racks, dollies, slatted shelves, or other clean surfaces in such a manner as to be protected from splash or other contamination. 15A NCAC 18A .1323 (c).
