Employees are only to eat outside of the permitted area. -EHS observed a member of kitchen staff with a vape in hand while standing in the kitchen. Vaping is considered the same as smoking when inside a permitted facility.

Pf -EHS observed the back handsink being blocked by a trash can and speed rack. These items were rearranged to allow access to the hand sink. Ensure hand sinks are never blocked. CDI

Pf -EHS observed multiple dishes throughout the drying rack with food debris still on the food contact surfaces. Ensure dishes are being thoroughly scrubbed to eliminate all debris. EHS will return within the next 10 days to verify that dishes are being properly and thoroughly cleaned.
40 0.50 Personal cleanliness No No No 2-402.11 - Effectiveness: Use head coverings, beard guards and clothing to restrain body hair from contacting exposed food, equipment, and utensils. -EHS observed one of the employees not wearing a hair restraints . Hair restraints must be worn when handling and preparing food.
44 0.50 Utensils, equipment & linens: properly stored, dried & handled No No No 4-901.11 Air dry equipment and utensils after cleaning and sanitizing. -EHS observed dishes stacked on the drying rack in a manner that causes water to pool in the rim of the plastic pans. Store at an angle to allow for dishes to completely dry before stacking. PIC stated that large number of pans were set to be removed from facility which will increase storage space for air drying.
47 0.50 Equipment, food & non-food contact surfaces approved, cleanable, properly designed, constructed & used No No No 4-501.11 Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted. -EHS observed the gaskets on the leftmost door of the prep unit and the door to the freezer being damaged. PIC stated that work orders are in the system for these gaskets to be replaced.
49 0.50 Non-food contact surfaces clean No No No 4-601.11 Food contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and accumulations. -EHS observed large accumulations of grease baked onto the pizza pans. Ensure these pans are being thoroughly scrubbed to eliminate as much build up as possible during wash rinse and sanitize cycles. Focus on the pans where grease is beginning to crack as these pieces could potentially flake off into food. 4-601.11 Non-food contact surfaces and utensils shall be clean to sight and touch. -EHS observed the storage rack for clean dishes with an accumulation of grease on the rack itself. Clean rack frequently enough to prevent build up.
54 0.50 Garbage & refuse properly disposed; facilities maintained No No No 5-501.113 Keep dumpster and other outside waste handling containers for refuse, recyclables, and returnables covered with tight-fitting lids or doors. -EHS observed both sides of the lid to the dumpster being open at the time of the inspection. Ensure after disposing of trash that doors are properly shut.
55 0.50 Physical facilities installed, maintained & clean No No No 6-501.11 Physical facilities shall be maintained in good repair. -EHS observed damaged floor tiles throughout facility. -EHS observed need of grout around the tiles in the 3 vat sink room. -EHS observed the light cover in the walk in cooler being cracked. Replace the light cover to ensure ease of cleaning. 6-501.12 Physical facilities shall be cleaned as often as necessary to keep them clean. -EHS observed vents throughout facility needing to be cleaned.
