4-501.11 Equipment maintained in good repair. Water is leaking from around the fan box in the walk-in cooler. water is being caught by a bucket and no food is being stored near the leak. Have the leak repaired. Replace the torn gaskets on the Pepsi cooler, walk-in cooler door, on both doors of the 2 door freezer and on the prep cooler. Repair/replace the panels on the inside of the doors of the small prep unit where they are broken and insulation is seen. Door is no longer easy to clean. Replace missing handle of the top lift door of the small prep unit.
4-101.19 Nonfood-contact surfaces of equipment exposed to splash, spillage, or other food soiling or requiring frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material. The wood on the bar and on the single use shelving must be stained or painted. The wood right now is raw wood. If something spills on the wood, the wood is very absorbent and moisture can cause the wood to rot. Some of the laminate is coming off of the hand sink vanity, exposing the particle board and water has been seeping into the wood. Replace the vanity if it cannot be repaired.
4-202.16 Nonfood-contact surfaces shall be free of unnecessary ledges, projections, crevices, and constructed to allow easy cleaning and facilitate maintenance. Eliminate rust on the metal shelving in the wall-in cooler. The top of the left stove is starting to rust. Remove rust and apply a appliance paint so that it is easy to clean. Most items are repeated items so full credit has to be taken. |

Clean the inside of the 1 door freezer, 2 door freezer and prep cooler. Also clean inside the large prep cooler that not longer works, but is used for storing bread. Clean inside ovens. Clean stove drip pans. Clean all gaskets in coolers and freezers as they are very dirty. Clean the all sides and inside the fryers where grease is accumulating. Clean under the grill. Clean the fan covers on the fan box in the walk-in cooler. Clean the inside and outside of the microwave. Clean both hoods and the hood filters. Repeat violation. Full credit taken again due to being multiple repeats. |

5-205.15 Maintain a plumbing system in good repair. The hot water at the 3 comp sink where dishes are washed is being turned off at the back. A spoon is no longer used, but the water still does not turn on and off at the hot water handle. This is not a proper repair. Repair the sink so the water is turned on and off at the faucet. There are no handles on the right faucet of the 3 compartment prep sink so that faucet cannot be used. Water cannot be turned on and off with the handles on the left faucet. Water is being turned on and off at the water lines. The cold water also is not turning off all the way. Repair so that both faucets are in working order. Repeat violation full credit must be taken. |

6-501.11 Physical facilities shall be maintained in good repair. Repair wall where paint is peeling next to the ice machine and next to the microwave. Replace all missing and broken floor tiles in the kitchen. Repair gap between the wall and ceiling in prep sink area and along the back wall near the ac unit. Holes in walls and ceilings allow areas where pest can enter facility and live. Eliminate any holes in walls and ceilings. Repair ceiling damage above fuse box near back door. Repair the damaged floor in both restrooms. The back of the women’s restroom door is damaged and rotting in places. Repair the door. Replace the door if it cannot be repaired.
6-501.12 Physical facilities shall be cleaned as often as necessary to keep them clean. Clean floors under and behind all equipment. There is a lot of grease building up on the floor under and behind the fryers. Clean walls throughout especially under prep table against the back wall and behind and next to the ice machine. Clean the ceiling. Clean floors next to the ice machine. Clean floors under racks in walk-in cooler. Full credit taken again due to being marked multiple times. Repeat. |
