General Comments

Inspection completed with Aaron Fennell on site. No signature due to COVID-19. Intent to suspend issued for records. Observed pH and main drain checks not being recorded every day. **NOTE** Observed 'dummy' covers installed (no suction) on walls below handicap chair and at cutouts where fountain sprayers used to be installed. These covers have expired. Replace/cover over prior to issuance of 2021 operations permit. **NOTE** Observed some areas within 18 inches of gate push bar with openings greater than 1/2 inch by 1/2 inch. Ensure no areas within 18 inches of push bar have access holes greater than 1/2 inch. Repair prior to issuance of 2021 operations permit.
