Floors, walls, and ceilings of all areas shall be kept clean and in good repair. 15A NCAC 18A .2410.- Observed water stain on ceiling tiles in hall way by cafeteria and office hall. Check for no active leaks, repair/replace ceiling tiles. Observed several AC ceiling vents cover with dust. Cleaning needed and keep vents clean. Observed holes on restrooms walls due to missing soap dispensers/mirrors, repair and keep all walls in good repair. - Head of custodian: Franck Kehi Tehe present during inspection

Detail cleaning and keep all hand sinks clean. Observed disrepair/missing soap dispenser from the wall in several restrooms. Observed several hand sinks with gap between sink and the wall. Repair needed. Keep all fixtures in good repair. Soap and individual towels or approved hand-drying devices shall be provided. 15A NCAC 18A .2409 (c).- Observed several empty soap dispensers in restrooms throughout the facility. Ensure to provide all hand washing supplies at all dispensers at all hand sinks.
