Pests shall be removed from the PREMISES at a frequency that prevents their accumulation, decomposition, or the attraction of pests. Observed a dead roach around the perimeter of the prep table on the left side of the stove. When cleaning, thoroughly remove all dead pest. 6-501.111 Controlling Pests: The PREMISES shall be maintained free of insects, rodents, and other pests. Observed live roach next to the front handwashing sink of wait station. Pest control record was not available; employee stated that possible service was either in March 2025 or July 2025. Note: if pest is found on food or food contact surfaces ( prep table, spoon, pots and pans etc.) facility is to be immediately suspended. Pest control was scheduled for May 25, 2026 but is rescheduled for a later date.

Observed deep grooves. 4-502.11 (A) Good Repair (C): A) UTENSILS shall be maintained in a state of repair or condition that complies with the requirement: Observed multiple white container with yellow lids are cracked and chipped. Observed ice scoop holder broken and chipped of plastic pieces.
