Observed many water damaged / stained ceiling tiles throughout facility. Cracked glass panes at front entrance door. Floor tile damage at sliding gate. Cracked window room 161. Frayed carpeting in reception area. damaged wall landing of 1st floor. Windows won't shut rooms 245 and 247.
