General Comments

Inspection conducted with Eddy Alvarez. Due to COVID-19 precautions, inspection was not signed by the operator. 7) Heavy scum buildup on tiles, plaster with green staining, plaster chipping. Maintain spa floor and walls clean and in good condition. 22) Operator had difficulty hearing me. Recommend adjusting microphone volume on emergency phone. 22) Unable to connect to emergency phone when called. Ensure that emergency phone is capable of accepting calls in the event of a dropped call. Will return to check in 2 weeks. Failure to correct this will result in permit suspension. 24) Flow meter heavily worn.
